Defect Template

I have come up with the following defect template to ensure the uniformity among the members who are part of the testing/validation team.
Summary
1.     Issue summary should be crisp [E.g. user with option A  is not listed under the report ABC]
2.     Pre-fix the summary with the Release Index, if any. [E.g. SFXRelease - ]
Description
Test Case ID:
Customer: , Browser name, Version:
Steps to Duplicate
Pre-requisites (If any): [E.g. Option A should be available for the customer for which the user for feature 1 is being created]
  1. Login nto as \ for the Role:
  2. Exact Navigation steps and data should be specified to replicate the issue [E.g. Click Manage->Create User->Enter necessary details etc]
  3. To reuse the data from the previous steps, quote the step number. E.g.
    1. Click Manage->Create User
    2. Enter necessary details->Select Option A
    3. Click Review and Save. Note the Option A
    4. Manage->workflow1->feature1->page2
    5. Select the item from step c.
  4. Attach screen shots (if any)
  5. When a defect is tested/re-tested update the comments with latest status or discussions with any dev/product/QA personnel
  6. Remarks can be added at the end of the defect description as a postscript
  7. Attach logs (If any)
  8. Mention the schema/table name/query for database details (if any)
  9. Mention the clearing of cache or cookies (if any)
  10. Performance details (If any)
  1. For all the UI related issues, click on the 'Configuration Tab, to select the Browser Type
  2. PS: The headers can be prefixed and suffixed with ‘*’ to bold them.
